If you do heme/onc at a larger academic center it seems like it's expected of you to devote quite a large portion of time to research.Heme/Onc: I find it more interesting than rheum and already have research in this field. However, I'm concerned with emotional burnout with the patient population and intensity of the fellowship as compared to rheum. Even after fellowship, I perceive heme/onc to be higher stress and work hrs than rheum, not sure if this is true.Speaking anecdotally, I don't know a single oncologist working that much. Part of a hospital, they will want you to see more patients. At my hospital, the heme/onc program tends to be about 50-55 hour weeks. Its a community program with plenty of other hospitals around. As far as vacation time, I can tell you my oncologist took 1 week vacations twice during the time I was getting chemo.

I am an IMG as well. Heme/Onc is a terrific field. We get to know the patients well, it is mentally stimulating, nothing is routine or monotonous. The cons are that it involves very serious illness. Many of us grow close to patients and it can take an emotional toll when patients die or become sick.
